The Foundation of Community Hospice & Palliative Care hosted a special VIP reception to commemorate the 40th anniversary of the St. Augustine/St. Johns County Hospice program and to unveil the newly remodeled Stephen R. Chapman Family Community Campus, located at 1355 U.S. Highway 1 South, St. Augustine, FL 32084.
Community leaders, long-time supporters, volunteers, and staff gathered from 5 to 7 p.m. to celebrate four decades of compassionate care for patients and families in St. Johns County. Guests enjoyed light refreshments, toured refreshed spaces, and heard remarks reflecting on the program’s history and the community partnership that has sustained it since its founding.
“We honored a legacy of care that began through the efforts of volunteers in our community 40 years ago. Their determination to bring hospice care to their fellow residents resulted in the expansion of services to St. Augustine and St. Johns County. We continue to provide this special care and support to our community every day in homes, and health care facilities and The Bailey Center for Caring at UF Health-Flagler,” said Susan Ponder-Stansel, President and CEO of Alivia Care, Inc. “The remodeled Stephen R. Chapman Family Community Campus is more than a beautiful building. It is a welcoming place for groups and organizations to gather, for members of our community to learn about important matters such as advanced care planning and caregiving, and for families to find grief support. It also serves as our patient care team’s home base as they deliver care to patients and families in this community we love.”
The Chapman Center’s conference room, boardrooms, and event space are now available for nonprofit groups, clubs, and civic associations to hold meetings. Community, civic, and professional organizations can reserve rooms for meetings, educational events, community outreach, and networking activities; the spaces include audiovisual support and access to a catering kitchen. In addition, Community Hospice & Palliative Care will offer community-benefit services, including grief support groups, advance care planning sessions, and other helpful community education programs, all free of charge. These offerings reflect The Foundation’s commitment to removing barriers to care and making a meaningful impact in the communities we serve. The remodeled campus is a visible reminder of the commitment to this community that began 40 years ago, and that Community Hospice & Palliative Care continues to honor.
