Beacon Medical Group Centennial Neighborhood Health Center is a patient-centered medical home, a team-based model of care led by a primary care physician, social worker, clinical pharmacist, nurse/medical assistant and case manager, who together provide continuous and coordinated care to improve the health of you and your family throughout your lifetime.
