Mission:

The mission of Hospice & Palliative Care Charlotte Region is to relieve suffering and improve the quality and dignity of life through compassionate hospice care for those at the end of life, palliative care for those with advanced illness, and through community education.

Hospice of Lincoln County is committed to enabling our patients to live as fully and comfortably as possible. Our philosophy of care is based upon treating the whole person, not just the disease.

Services:

Hospice of Lincoln County is a program of Hospice & Palliative Care Charlotte Region, an independant, not-for-profit health care organization, which provides a full range of services for patients with life threatening illnesses and supportive services for those who share their lives. Our interdiciplinary team of professionals and volunteers addresses the needs of the whole family, so that all involved may experience life as fully as possioble during the patient's illness.

- Intermittent Skilled nursing visits.

- 24 hour on call nursing for emergencies.

- Management of pain and other symptoms to ensure the patient's comfort.

- In-Home Aide assistance with bathing and other personal care needs.

- Medical social work visits for support and advocacy.

- Chaplain services for spiritual care.

- Expressive arts therapy is available to children and adults and provides a safe and structured setting in which to explore and appropriately express thoughts and feelings about grief and loss

- Bereavement support for family and loved ones experiencing grief and loss, a service also extended to the community at large

- Trained volunteers for respite, companionship, emotional support and other needs.

- Comprehensive case management and coordination of core medical services with patient's physician and the Hospice of Lincoln County's Volunteer Medical Directors.

- If needed, hospital care is coordinated by the Hospice of Lincoln County team Liaison with appropriate community resources for other necessary services.

- Community Education through our speakers bureau and community outreach services.

- Services are offered in long term care facilities, including assisted living and retirement centers, through our Hospice Alternative Residential Team (H.A.R.T.).

- The Kids Path children's program offers specialized health care and supportive services to help families cope with the serious illness of a child.
