Care Management

LifeSpan Resources serves as the single point of entry for a variety of supportive services for frail older persons and individuals with disabilities who have multiple needs. The Care Manager helps individuals obtain the supports needed to live as independently as possible in their community, by finding and coordinating available resources and services to meet the individual's needs.

Caregiver Support

The Family Caregiver Support Program is designed to benefit you in your caregiver role. Services are designed to meet the individual needs of caregivers and their situation. This program connects you with the right information and available community resources for your particular caregiving needs. Services include information and assistance, respite care and other supplemental services.

Nutrition Program

LifeSpan Resources provides a hot mid-day meal Monday through Friday to senior citizens at nutrition sites in each county and to the homebound. Reservations should be made 24 hours ahead by calling the Nutrition Site.
