Tulare Community Health Clinic is a nonprofit Federal y Qualified Health Center. Its mission is to provide comprehensive, companionate high quality health care for people living in the communities we serve and our vision is to build healthy communities by providing the utmost in comprehensive patient-centered care. We believe that it's not when a patient walks in to our door that we can help them with their care but to also prevent health issues before they even come through our doors.  This is why we have an outreach team whose focus is to go out and educate on health, nutrition and prevention.  The outreach staff works closely with the clinic, families, individuals and community services to promote health and preventive disease and injury by visiting patients in their homes and providing basic health assessments. This is accomplished by working together with all health professionals who serve the patient in the clinic setting. TCHC has an outreach team that participates and conducts health fairs in the community. They also participate in workshops, trainings (i.e. collision, abuse) to be involved and educate on community needs. Outreach is intended to increase access to primary and preventative health care and improve the health status of the underserved population in the service area of Tulare Community Health Clinic.

We also partake in community events, health fairs, Local partnerships; we also use our School base health Center Vans that are at our local schools, Migrant Camps, Our consortia's (Central Valley Health Network, California Primary Care Association), Promotoras and our outreach team that is also working with Covered California.
