Community Hospice & Palliative Care Recognizes National Hospice and Palliative Care Month

Each November, communities nationwide come together to observe National Hospice and Palliative Care Month. This month-long recognition highlights the importance of compassionate, person-centered care and the dedicated professionals who bring comfort and dignity to patients and families at one of life's most meaningful stages.

 

For more than 46 years, Community Hospice & Palliative Care has been proud to honor its interdisciplinary care team, including providers, nurses, hospice aides, social workers, chaplains, and volunteers, who deliver expert medical care alongside emotional and spiritual support. This team approach ensures that patients and families never feel alone, whether care is delivered within a hospice inpatient center, a nursing facility, or in the place many patients most want to be, wherever they call home.

 

When Gwen Smolik reflects on the final months of her husband Dennis's life, she recalls both the deep love they shared over 61 years of marriage and the incredible support they received from Community Hospice & Palliative Care. Dennis was a devoted husband, father, and grandfather, known for his intelligence, humor, and passions for fishing and trains. He never missed a moment of his son's activities, and together, Gwen and Dennis built a life filled with travel, sports, and family memories.

 

When Dennis became ill, Gwen knew she could not walk the journey alone. She had learned about Community Hospice through outreach in her community and turned to them when the time came. "I cannot imagine what I would have done had Community Hospice not shown up when they did," said Gwen. "They were there from the beginning, the middle, and the end."

 

Dennis received care at home from January to April 2025, surrounded by the people and the place he loved most. Within a day of enrollment, his home office had been transformed into a space equipped with everything needed to care for him. What stood out to Gwen was not just the medical expertise but the humanity of the team. The social worker, Julie, brought joy and connection to Dennis, and the hospice aide, Adrianne, had a way of gently breaking down his resistance with patience and kindness. Gwen also found strength through the reassurance and guidance she received from nurses and staff, who answered her questions, provided equipment, and ensured she felt confident in caring for her husband.

 

"They guided me step by step," said Gwen. "They helped me see I was doing the right thing, and they made me feel not so alone."

 

Gwen’s story is just one example of how hospice care brings comfort, dignity, and peace of mind to families during life’s most difficult moments.
