About Us:

Alamosa Family Medical Center has been committed to your health and wellness by delivering high quality care, since 1988. The Alamosa Family Medical Center (AFMC) is part of Valley Wide Health Systems, Inc. which was established in 1976. This clinic and the Valley Wide Pharmacy are centrally located in Alamosa, directly across the street from the Adams State University Nielsen Library, and serve all surrounding communities of Rio Grande County, Conejos County, Costilla County and Saguache County.

Mission Statement:

To provide medical education and training to residents in a rural accredited Family Medicine residency program while providing high-quality, safe, effective and integrated health care services to all with special consideration for the medically underserved populations.
