CNHC is a home care program to assist terminally ill patients and their families.  We provide care through a team approach, where staff and volunteers work together to fulfill hospice goals and support each other.  The hospice philosophy of care focuses on addressing the physical, emotional, spiritual and financial needs of the patient and family.  The support continues for the patient's family and caregivers through bereavement support for up to 13 months after the death of the patient.

We specialize in pain and symptom control, emotional and spiritual support, patient and caregiver teaching, and bereavement support groups and services.  All services are available 24 hours a day, to meet the needs of the patient and family.  The county coverage areas include:  Cambria, Bedford, parts of Indiana, Westmoreland, Somerset and Blair.

The team consists of skilled nursing, home care aides, medical social services, counseling services, spiritual care, volunteer services, bereavement care, nutritional counseling, physical, occupational and speech therapists.  Staff and volunteers receive hospice training and orientation.  All visiting staff are required to have Criminal Background Check, FBI Clearance, Child Abuse Clearance, physical and drug screen.

CNHC provides services through the Medicare/Medicaid Hospice Benefit and is able to bill other insurance providers through their Hospice Benefits.  Through the Hospice Benefit, CNHC accepts financial responsibility for services related to the patient's terminal illness.  This includes 24-hours staff availability, as well as providing illness-related pharmaceuticals, medical supplies, durable medical equipment, respite care and short-term hospital care.
