On May 14, 1876, at Flat Creek, Gadsden County, after preaching by Elder J.T. Mann whose text was 'Go Ye into all the World and Preach my Gospel' the following named brethren and sisters vez: (namely) I. N. Shepard, Joshua Shepard, J. Trussel, J. Johnson, H. Poythress, Wm. Johnson, E. Crawford, David Hall (black), Sallie H. Shepard, Sara S. Shepard, Nancy Trussell, Mary Poythress, Grace Ann Smith, S. J. Smith, M. O. Havens, Anne E. Fields, W.J. Crawford, Mary Smith, Martha Smith, Elizabeth Fletcher, C. Fields, Milly Hall (black), Melinda Dancy (black), Elizabeth Tolar, Margie Holt, and Rhonda Butler, having been dismissed by letter for Providence Baptist Church by request, came forward and extended the right hand of fellowship and after the dedicatory Prayer, the Bible was presented and a charge given to read, pray and endeavor to learn our duty from its pages and was declared to be an Orthodox Missionary Baptist Church of Christ after adoption of the articles of faith subscribed to by the Florida Association.  A copy of the covenant and Decorum was read and adopted with the privileges of making such changes as thought to be advisable.  Naming the church and electing Deacons and Clerk were deferred till the next meeting.".
