Horizon Care Center provides long-term skilled nursing care for 280 residents. Located on the Rockaway Peninsula in Arverne, New York, only a few short blocks from the ocean, the facility is easily accessible by car from all five boroughs and Long Island and is near bus and subway routes.

Horizon Care Center's resident community is diverse in background, culture, religion and age, and the care provided meets a broad range of needs. Some residents require only partial assistance with the activities of daily living, while others require intense treatment and total personal assistance.
