Rogue Community Health (formerly Community Health Center) is a nonprofit patient-centered medical home that offers a wide-range of primary and preventive health services to families and individuals of all ages and backgrounds. Our dedicated, patient-centered team of providers, nurses and support staff provide a continuous source of high quality, comprehensive and affordable medical care to thousands of Jackson County residents.

Rogue Community Health clinics and school-based health centers are recognized by Oregon Health Authority as Tier 3 Patient-Centered Medical Homes, the highest level of recognition available. Ashland, Butte Falls, Medford, and White City clinics are recognized by the National Committee for Quality Assurance (NCQA) for demonstrating that our practice and clinicians value quality health care delivery and the latest clinical protocols to ensure that our patients receive the best care at the right time.
