The Center for Health Education, Medicine, & Dentistry (CHEMED) is a Federally Qualified Health Center dedicated to providing primary health care, disease prevention, health education, case management services, and social service referral to all residents of our area to promote and maintain healthier lifestyles. CHEMED opened in February 2008 as a division of the Lakewood Resource & Referral Center (LRRC) to offer health care and education to the Lakewood, NJ community.
CHEMED provides comprehensive preventive and primary care medical, dental and behavioral health services for adults and children, as well as health education, and nutrition services.
These services are available to all patients regardless of the ability to pay. CHEMED participates in all Medicaid and Medicare plans as well as private insurance coverage. A sliding fee scale is available for our uninsured patients.
CHEMED’s primary care Adult services extend beyond sick visits and include a focus on preventative care, such as screening and assessment for an assortment of cancers, cardiovascular health, and behavioral health, including substance abuse. Preventative services such as assistance with smoking cessation and the development of strategies to manage asthma are provided to promote the health of the community. For those with hypertension or other forms of coronary artery disease, diabetes and/or other potentially debilitating diseases, specialty services coupled with nutritional counseling are available within CHEMED.
CHEMED Pediatric services include all necessary immunizations and screening and preventative services. CHEMED also has a full Behavioral Health department at our facility offering counseling for adults and children. CHEMED is committed to ensuring the well-being of all of our patients; as such CHEMED provides quality education, screening, and intervention for a variety of behavioral health issues. Our facility also contains a full state-of-the-art Pediatric and Adult Dental Department.
Our physicians promote the use of preventive services and compliance with treatment plans and follow-up appointments. CHEMED’s delivery model of providing both primary care medical and dental services in one treatment setting ensures coordination of care. Our social service department ensures that our patients receive access to available services that assist with the financial needs inherent I their medical care.
Over the next year, CHEMED will complete construction of our Women’s Health and Radiology Departments, with on-site Mammography and ultrasound.
